BRAND NAME
The common brand name is CLOPIXOL
. ALthough zuclopenthixol deconate is used in more chronic patients and is administered as an IM but, Zuclopenthixol acetate, also known as Acuphase is used in acute treatment of psychosis and is administerd as an IM injection. HOwever, zuclopenthixol dehydrochloride is the oral form and is used with patients who are compliant with their medication
DOSAGE
Normal dose range from 50 mg 4 weekly to maximum of 600mg weekly. In general, the lowest effective dose to prevent relapse is preferred. The dose should be reviewed and reduced if side effects persists to occur.
SIDE EFFECTS
Some of the common side effects that could occur are abnormal gait
abnormal muscle movements or problems controlling muscle movement
nausea
vomiting
confusion
constipation
convulsion
depression
dry mouth
sedation
tirednes
weight gain
stomach ache
speech problem
tremors.
CONTRAINDICATION
Some of the known contraindication of Zuclopenthixol Decanate are Known hypersensitivity to the thioxanthenes.
The possibility of cross-sensitivity between the thioxanthenes and phenothiazine derivatives should be kept in mind.
Known hypersensitivity to any of the excipients of the particular Clopixol presentation Acute alcohol, barbiturate or opiate intoxication.
Circulatory collapse, depressed level of consciousness due to any cause, coma, suspected or established subcortical brain damage,Blood dyscrasias,Phaeochromocytoma.,
Leucopenia and/or previous agranulocytosis.
SPECIAL NURSING CONSIDERATIONS
Some of the special considerations that need to be kept mind are to monitor the signs and symptoms neuroleptic malignant syndrome such as
fever
hypothermia
altered concious state
profuse sweating
dyspnoea
Neuroleptic malignant syndrome s considered as medical emergency and need to acted upon very quickly as it may even lead to death
Pt needs to tell their GP or doctor about the other medications they are taking as it may interact with clopixol such as adrenaline, anti chlorgenics, anti diabetic, antihypertensives, CNS depressants
Patients should be warned about alchohol consumption as it may effect the drug serum level and lead to further complications.
EDUCATION
Patients/families/cares being commenced on zuclopenthixol deconate ( Clopixol) needs to be educated.
Ambulant aptints should not drive or operate any type of machinery.
Patients should be forewarned and reassured concerning the possible occurrence of extrapyramidal
Patients should be instructed to report any soreness of the mouth, gums, throat or other symptoms which may indicate suppression of the immune system symptoms.
More over patients should be warned that long term use of "Clopixol" may effect Hepatic function. Reduced Renal function is one of the other disadvantages of long term use of the drug
